A doctor and pharmacist in Oklahoma City have been charged with manslaughter after administering an unnecessarily large dose of methotrexate to an elderly patient. The patient was supposed to take 20mg of the drug every 7 days, but prosecutors found that Alexander Frank, M.D., mistakenly prescribed a 20mg dose every day for 7 days and Justin Lee, Ph.D. claimed to have ignored it.
